Job summary

This is a very exciting opportunity for an experienced Programme Co-ordinator to support the development, implementation, continuous monitoring and improvement of the Active Together Programme.

Airedale NHS Foundation Trust are working collaboratively with Yorkshire Cancer Research and Sheffield Hallam University to implement the Active Together model to individuals diagnosed with cancer across Airedale, Wharfedale and Craven.

The Active Together programme will run alongside the existing diagnostic and treatment pathways to ensure patients are optimally prepared for treatment and beyond. It aims to maximise resilience to treatment, improve longer term health, minimise side effects and empower patients to be partners in their healthcare. Patients will receive individual assessments and be triaged into a support pathway suitable to their needs.

We are a small and friendly team based at Skipton Hospital which includes Physiotherapists, a Dietitian, and a Psychologist.

Main duties of the job

As programme co-ordinator you will support the Therapy manager in developing and delivering the service with key stakeholders across Airedale, Wharfedale and Craven with main responsibilities for the data collection and reporting.

The role also involves the smooth running of the appointment system and communicating effectively with team members and patients either face to face or via the telephone. The often requires skills of tact and persuasion and adopting a flexible patient centred approach at all times.

The post holder will also have a responsibility for the programme data collection and data reports, working closely with the Active Together team, Sheffield Hallam University and Yorkshire Cancer Research to ensure the project data is reported in line with the project outcomes. The role requires knowledge and experience of developing and delivering project marketing plans and working with partners to engage and promote the service.

Assisting in the production of project reports for all relevant project partners

Processing and collating the Active Together date from NHS systems and key stakeholder systems delivering the programme.

Handle sensitive queries in writing, email, by telephone or in person from colleagues, patients, Trust employees and other health related organisations in an effective, sensitive manner as a representative of the Trust, to reflect a positive corporate image.

  • Relevant qualification or expert competence in use of IT systems such as Microsoft office, eg ECDL/OCR.
