Outreach and Digital Pathway Nurse (Band 6)

Job summary

The post holder will have a pivotal responsibility in identifying, supporting, and facilitating the early discharge of patients through the utilisation of remote monitoring technology.

Collaborate with multidisciplinary teams across both acute and community settings will be essential to determine patient suitability for early supported discharge, while ensuring safe, effective, and person-centred care delivery.

This role is integral to admission avoidance, minimising hospital length of stay, and enhancing patient outcomes via technology-enabled care.

Participate actively in multidisciplinary team (MDT) meetings and discharge planning sessions.

Coordinate the safe onboarding process for patients, ensuring comprehensive understanding and engagement.

Develop and integrate innovative digital pathways consistent with the Digital Care Hub Strategy.

Work closely with Digital Care Hub Team to ensure high quality referrals, accurate handovers and safe onboarding of patients onto remote monitoring pathways.

Main duties of the job

Participate in daily huddles to identify patients suitable for early supported discharge using remote monitoring technology.

Identify and assess patients suitable for early supported discharge and remote monitoring

Provide day to day coordination of outreach activity, ensuring effective communication between acute, community and Digital Care Hub team.

Coordinate safe onboarding of patients, ensuring understanding and engagement.

Support safe transition of patients from hospital to community setting with appropriate monitoring plans in place.

Provide patient and carer education on remote monitoring equipment and digital engagement

Act as a clinical liaison between the Digital Care Hub, community teams and acute services to ensure seamless communication and continuity of care.

Lead on the development and implementation of new digital pathways that align with Digital Care Hub services.

Develops good Trust wide working relationships and communicates effectively with nursing colleagues and members of the multi-disciplinary team.
