Job summary
We have an amazing opportunity to work differently to support our Craven Residents. The Team Leader for Enablement and IMC will work across community settings, predominantly within the Craven Area, but also supporting the wider Community Rehab Team as required . Providing support and leadership, along with existing teamleaders, to lead and manage an integrated team of occupational therapists, physiotherapists, and support staff. To enable Home First discharges from Hospital Settings and Provide Rehab services in our Craven Intermediate care beds. The community rehab team provides specialist assessment, treatment, and rehabilitation in intermediate care beds and in patients own homes. Blending boundaries and developing intermediate care roles with the Local authority.
This role is key in facilitating safe and timely hospital discharges local hospitals. In conjunction with Craven Enablement services as well as ensuring IMC beds are used to best effect and are supported to ensure best use of resource.

Main duties of the job
To undertake the role of the Team Leader; managing,planning, coordinating, delivering and evaluating therapyplans as part of this broad MDT. Deputising for theService manager as required
Provide support and leadership, along with existing teamleaders, to lead and manage an integrated team ofoccupational therapists, physiotherapists, and support staff
To ensure the team provides high quality assessment,treatment and rehabilitation to people in their own homesand IMC beds
To act as a source of expertise and provide an advisoryservice to other ANHSFT therapists and members of theMDT in the management of Home First and IMC patients.
To lead in the advanced assessment and treatment ofpatients within the speciality who may have complex andchronic presentations, and determine clinical diagnosisand any therapy treatment indicated, and to maintainrecords as an autonomous practitioner.
To provide expert training and education for all grades ofstaff and other health professionals in the therapeuticmanagement of community rehab patients.
Provide effective student training and development.
Provide a key role in partnership working with patients,carers, the MDT, bed managers and wider statutory,private and voluntary sector services
