Job summary
An exciting opportunity has developed to join this district nursing team to lead on the developments within District Nursing. This will include disease management as well and exacerbation care planning and involve supporting the team to develop the skills and confidence in caring for this group of people. You will use your leadership skills in deputising for the team leader when required and also support the function of practice assessor for specialist practitioner district nursing student's as part of your role.
You will work with the District nurse team on a day to day basis supporting safe caseload management and promoting high quality nursing care.Ideally the candidate will have advanced level skills although we are keen to meet those keen to develop these skills.
Main duties of the job
The post holder will provide support education and supervision to the district nurse team in the care of people with complex care needs to enhance standards of care and coordination.
The post holder will lead on service planning, delivery, redesign and performance management of the service within their area of responsibility. The post holder will work closely with their local Clinical Commissioning Group to understand the health needs of their registered population.
