This is an exciting opportunity for an enthusiastic, motivated person to join our established integrated intermediate care team. The Collaborative Care Teams have undergone developments and in the CQC report, the Collaborative Care Teams were described as an outstanding example of community working.
Main duties of the job
The service aim is to support patients with a health related crisis at home to try avoiding an acute hospital admission, also patient who require out of hours District nurse input. We provide intervention for people at home in virtual ward beds in the community.
The service received referrals from a wide range of health and social care professionals. The post-holder will be required to work flexibly and collaboratively. We actively support blurred boundary working within the Calderdale Framework.
