Job summary

This is an exciting opportunity to join the respiratory specialist nursing team. If you are looking for something new, to develop your skill set, whilst working flexibly and agile; this maybe the role for you! The post is advertised as band 7 dependent on experience. However, this could be offered as a band 6 development post to band 7 following training and competence. This will be discussed at interview.

We are an integrated service assessing/reviewing and supporting inpatients and those in the community setting, offering home visits and specialist clinics delivered both virtually and face to face. This is a diverse role and requires working as an autonomous practitioner, managing a caseload of patients, participating in MDT's, patient and carer education, teaching other professionals, participation in national audits and service development.

Main duties of the job

  • Assessing and reviewing patients admitted to hospital with an exacerbation of their respiratory condition.
  • Assessing and reviewing patients in the community setting who are symptomatic with their respiratory condition to optimise treatment.
  • To provide advice/support or signpost appropriate services to patients who contact the Respiratory Advice Line.
  • To identify suitable patients for referral into the existing pulmonary rehabilitation scheme and provision of specialist education sessions to the existing pulmonary rehabilitation scheme.
  • To participate in the National Respiratory (COPD and Adult asthma) Audit Programme (NRAP)

  • To use established strategies for managing patients with complex respiratory needs in community settings, facilitating earlier discharge and avoiding unnecessary re-admission to hospital.
  • To be a source of expertise, knowledge and support and promote integrated working with other professionals and members of the multi-disciplinary team (MDT) in primary and secondary care working with long term respiratory conditions.
  • To ensure provision of a high standard of assessment, intervention and management of people with a respiratory condition including those with complex and chronic presentation.
  • To provide skilled training and education for all grades of staff and other health professionals in the therapeutic management of respiratory patients.
  • To maintain and develop effective integrated links across boundaries with General Practitioners, Hospital Consultants, Ward Staff, Practice Nurses, District Nurses, Advanced Nurse Practitioners, Intermediate Care Teams, Community Matrons, Oxygen Services, Community Hospitals and Community Respiratory Clinics.

  • NMC/HCPC current registration
  • Evidence of accredited post graduate courses/CDP

Desirable

  • Post-graduate Respiratory Course including COPD or willing to undertake

  • Independent Non-Medical Prescriber or willing to undertake
