Main duties of the job
The Band 7 Lead Mental Health Nurse will support the complexity of the caseload, they will oversee and manage the risk, they will prescribe and take responsibility for developing the mental health knowledge and links between the community teams. The Mental Health Lead Nurse will deliver evidence-based community mental health care, supporting the most complex cases whilst providing support/education/clinical supervision to the wider Community Services teams.
They will promote and maintain the highest attainable standard of nursing care in a community setting, ensuring information on diagnosis and care planning is appropriately documented for continuity and transparency. They will oversee and develop personalised pre-diagnostic support with people awaiting diagnosis, improving awareness to increase identification and potentially increase diagnosis rates, and embed best practice guidance within all aspects of work.

An exciting opportunity has arisen for an experienced mental health nurse to join our established integrated intermediate care team. The Collaborative Care Teams have undergone developments and demonstrate an outstanding example of community working.
The aim of the service is to provide an urgent community response to avoid unnecessary hospital admission, to support patients to remain at home where ever possible and facilitate safe hospital discharge, the service receives referrals from a wide range of health and social care professionals.
As a member of the Collaborative Care Team the post holder is responsible for carrying out a comprehensive mental health assessment of patients in crisis at home and responding with the most appropriate interventions.
The post holder will work closely with acute and community mental health teams in the area as well as providing educational opportunities for members of the Collaborative Care Team's and wider community teams.
The Collaborative Care Teams are passionate about the service and take real pride in putting patients at the heart of all we do, this is reflected in our collaborative approach to teamwork. The Collaborative Care Teams include community advanced clinical practitioners, registered nurses, physiotherapists, occupational therapists, healthcare support workers and assistant practitioners.
