Job summary

We are looking for a Heart Failure Nurse Specialist to join our community nursing team.

  • To provide care for a caseload of chronic heart failure patients which aims to improve their quality of life and reduce unnecessary hospital admissions.
  • Be responsible for providing management of chronic heart failure, giving expert advice, education and delivering evidence based individualised care for patients and their carers and other health care professionals across primary, secondary, palliative and social care.
  • Empower patients to take an active role in their care and promote self-management.
  • Manage nurse led heart failure clinics across the locality and undertake home visits.
  • To work as a specialist nurse in collaboration with the Heart Failure Clinical Lead to improve the management of heart failure patients through specialist education, training and initiation of core disease modifying medications, in line with national / local guidance.
  • To work autonomously assessing individual patient needs, initiating investigations, utilising advanced clinical assessment skills and knowledge, to prescribe and optimise heart failure medications to improve quality of life, morbidity and mortality rates.
  • To provide clinical leadership and line management for staff within the heart failure nursing team.

Main duties of the job

  • To assess, plan, provide and evaluate nursing care to meet the care needs and case manage individuals with chronic heart failure with a reduced ejection fraction.
  • To work in partnership with patients (and carers) to empower them to make informed choices about their health and care.
  • To review the prescribed medication regimen to ensure that patients receive appropriate pharmacotherapy in effective doses.
  • Develop and lead on incorporating evidence-based practice into the management of skilled holistic care for patients with heart failure.
  • Accountable for organising own workload within working hours, prioritising according to clinical need, urgency and appropriateness.
  • To ensure the service keeps pace with the research evidence through identification, application and dissemination of research findings relating to heart failure management.
  • To monitor, evaluate and audit the service at regular intervals, as part of an agreed programme to ensure both a high standard of care and the effectiveness of the service to improve health outcomes.
  • Manage programmes of specialist care effectively through working in partnership with others.

  • Registered nurse with current NMC registration
  • ENB 998 or equivalent teaching and assessing qualification
  • First Level Degree in health-related subject
  • Heart Failure / Cardiology or relevant qualification

Desirable

  • Post graduate qualification at Masters level (or willingness to undertake)
  • Non Medical Prescriber (or willingness to undertake)
  • Advanced clinical assessment skills

Experience/Knowledge

Essential

  • Knowledge of a range of cardiac nursing issues
  • Knowledge of current ESC, NICE guidelines for CHF & ACS
  • Knowledge of current issues affecting the NHS, Community Services
  • Significant post registration including clinical experience working with cardiology patients.
