Digital Care Hub Transformation Project Lead (Band 6)

Job summary

The post holder will play a vital role in supporting the Digital Care Hub (DCH) Manager to transform and implement both new and existing services within the Digital Care Hub. The role will involve working with a wide range of stakeholders including: DCH clinical and leadership team, Airedale NHS FT, teams from across other provider organisations (Bradford Teaching Hospitals FT, Bradford District Care Trust, Primary Care), ICB programme management and commissioning team. Embedding a culture for innovation and transformation within the DCH will be an essential part of the role.

Main duties of the job

  • Actively supports the DCH Manager in the development and implementation of new services, leading on delegated areas of the initiative
  • Actively supports the DCH Manager in the review and improvement of existing services, leading on delegated areas as required
  • Provide efficient and effective project management support across all DCH developments to enable transformation and service redesign across the BDC Place
  • Plan, co-ordinate and monitor the delivery of projects using proven project or programme management methodologies such as PRINCE2 or Managing Successful Programmes.
  • Ensure robust project management arrangements are in place. Set up control and assurance processes including identification of key milestones, benefits realisation reporting, management of risks and issues, highlight reporting, exception reporting, contingency planning.
