Job summary
The Post Holder will support the development and continued implementation of the Gold Standards Framework (GSF), promoting Advanced Care Planning (ACP) throughout adult services within Airedale General Hospital.
The post holder will support medical, nursing staff and AHP to identify patients with life limiting conditions. Promoting the GSF and the importance of ACP.
Main duties of the job
The post holder will be working under the supervision of the Palliative Care Team Leader to co-ordinate and prioritise education and training for relevant health professionals.
The post-holder will support the implementation of education, training and evaluation of the GSF and ACP throughout adult services with Airedale General Hospital.
In collaboration with all relevant stakeholders they will demonstrate consistent, measurable improvements for patients with life limiting conditions (and carers) across tumour specific and chronic disease groups.
The post holder will work in partnership with nursing, medical and AHP colleagues across departmental and organisational boundaries.
The post-holder will collect, collate and undertake ongoing audit and feedback to governance and clinical forums which will be used to inform and underpin clinical practice and service development relating to end of life care.
