Job summary

We have an opportunity for highly motivated, enthusiastic and dedicated Senior Pharmacy Technician to join our multi-disciplinary award-winning team at Airedale Digital Care Hub. This would be an ideal opportunity for an experienced, registered Pharmacy Technician with excellent communication skills to develop their knowledge and practice around medicines optimisation in frailty. We will support the successful candidate with their introduction into this role, and with their future professional development.

Duties include maintaining systems for patients requiring medication reviews, medication reconciliation and reviews in line with competencies, analysing information, proposing options to optimise pharmaceutical care and delivering training to care home staff.

The successful candidate will have a BTEC in Pharmaceutical Science and NVQ Level 3 in Pharmacy Services or equivalent qualification and must be a Pharmacy Technician registered with the General Pharmaceutical Council. They will have excellent interpersonal skills, be good at managing their time, be flexible and able to work independently and be a team player.

Main duties of the job

Support the Digital Care Hub (DCH) pharmacists in delivering and evaluating medicines optimisation services to patients living in care homes. Maintain and implement systems for cohort of patients requiring medication reviews. Undertake medication reconciliation and reviews in line with competencies, analysing various information sources and proposing options in order to optimise pharmaceutical care. Deliver training on medication management systems within care homes to optimise safety and minimise waste. The post holder will support the delivery of a high quality medicines optimisation service, under the direction of the line manager who will assess the competencies of the post holder and ensure standardisation of activities.

The post holder will be managed by the Digital Care Hub Pharmacist Lead and provide a pharmacy technical link between the DCH clinical teams and care home teams, taking the lead in assessing and optimising patient's own drugs, patient counselling and education, training for care home staff and audit. They will be expected to pass in-house competency assessments and be willing to participate in the Health Education England Medicines Optimisation course if not already completed.

  • Maintain and implement systems for identifying cohort of patients requiring medication reviews, including residents at risk of medication related falls, inappropriate polypharmacy and high anticholinergic burden.
  • Follow documented departmental Standard Operating Procedures and all other related hospital policies. Recommend and implement changes to departmental SOPs. Be responsible for keeping up to date with changes to standard operating procedures and policies.
  • Reconcile medicines between GP and Care Home records
  • Undertake medication reviews in line with competencies and standardised procedures, analysing various information sources and proposing options in order to optimise pharmaceutical care and minimise harms.
  • Support audit of medicines management systems within care homes to optimise safety and minimise wastage.
  • Submit data using GP clinical systems and other relevant software applications.
  • Submit data on template questionnaires to monitor activity.
  • Provide technical advice on medicines related safety and effectiveness issues for DCH staff, care home staff and other health professionals in health and social care.
  • Support other health professionals e.g. care home staff and other health professionals by delivering medicines management services to patients in vulnerable groups and with complex medicines needs.

Counsel and advise patients or carers on the correct use/dosage, side effects, storage and any other relevant information about their medicines. Take accurate medication histories. Fill in templates and questionnaires for multi-professional use. Handle complaints and queries.
