Job summary
The Single Point of Access (SPOA) is the first point of entry into Adult Community Services. Itbrings together Community Care Teams, ARCH (Acute Response and Rehabilitation in theCommunity and Home), and UCR (Urgent Care Response). SPOA plays a vital role incoordinating and managing referrals across these services through efficient triage, ensuringpatients receive timely and appropriate care. This position is central to ensuring patients receivethe right care at the right time. By coordinating referrals and assessments, the SPOA teamhelps reduce unnecessary hospital admissions, supports timely discharges, and ensurescommunity services are delivered efficiently and compassionately
Main duties of the job
Act as the first point of contact for referrals into Adult Community Services.
Undertake detailed triage of patient healthcare needs to ensure safe and effectiveallocation.
Manage both urgent and routine referrals, responding decisively to urgent calls whileensuring routine cases are triaged effectively.
Support admission avoidance and facilitate timely hospital discharges throughresponsive referral into community services.
Liaise with a wide range of community services including:
- Community Nursing Teams
- Reablement Services
- Urgent Care Response (UCR)
- Hospital at Home
Essential Skills & Experience
Knowledge of urgent care and community-based services.
Ability to manage referrals and coordinate across multiple teams.
Strong decision-making skills, particularly in urgent situations.
Excellent communication and organizational abilities.
Experience in supporting admission avoidance and hospital discharge pathways.
Working Pattern
Flexible working required across 7 days per week to meet service needs.
