We are looking for an Advanced Clinical Nurse Specialist to join our Rapid Diagnostic Service. There is an opportunity for an experienced nurse with relevant experience to commence this post in a training role and Band 6. Please see both Job Descriptions.
The Rapid Diagnostic Service (RDS) aims to enable a fast diagnosis for all patients with the highest level of care, fairness and compassion. The RDS Advanced Clinical Nurse Specialist (CNS) will work with the existing RDS Nursing team to rapidly triage and assess patients referred into the Serious, Non-specific Symptoms Pathway (SNSP), plan and implement care across a variety of settings, providing highly specialist advice working as part of the multi-disciplinary team (MDT).
Main duties of the job
To practice at an advanced level demonstrating in-depth knowledge and competence inclinical assessment and examination, initiate appropriate investigations and interpreting the results as an autonomous practitioner ensuring evidence based provision of highly specialistadvice and support.
To communicate highly sensitive and complex information including diagnosis and/or prognosis to patients and carers.
To promote the philosophy of patient centred care by ensuring each patient is given acoherent and comprehensive plan of care and treatment following assessment throughouttheir pathway which is evaluated at key points. The plans will be developed in conjunctionwith patient and carer and the multi-disciplinary team.
To maintain and share accurate records to demonstrate appropriate patient care and followup as appropriate enabling accurate audit to be carried out.
Act as the patient's advocate when informed discussions may lead to choices being madeconcerning treatments, intervention and care options. Be able to impart and provide specialistsupport where patients are being given very sensitive and complex information.
