Cancer Care Co-Ordinator - Haematology

Job summary
We are seeking a compassionate and organised individual to join our Haematology Nursing team as a Cancer Care Coordinator. In this vital role, you will support patients throughout their cancer journey, acting as a key point of contact for patients, carers, and healthcare professionals. You will help ensure timely access to care, information, and support, contributing to a seamless and personalised care and support.
Main duties of the job
Main Duties
- Coordinate care for patients on a cancer pathway under the supervision of Clinical Nurse Specialists.
- Telephone triage
- Act as a liaison between patients, families, and multidisciplinary teams.
- Triage patient enquiries and escalate concerns appropriately.
- Support personalised care planning through holistic needs assessments
- Maintain accurate patient records and assist with clinical administration.
- Promote patient self-management and signpost to appropriate services.
- Contribute to service improvement, audits, and patient feedback initiatives and education .
- Work collaboratively within a dynamic team to enhance patient outcomes and experiences
- Communicate sensitively handling telephone enquiries from patients and their families.
You will need to have a professional manner, excellent communication skills and an empathetic nature. Attention to detail, interpersonal skills and an adaptable approach to work are essential as are literacy and IT skills.
