Job summary

We require a motivated and dedicated nurse to work within the integrated Palliative Care Team in Harrogate and District NHS Foundation Trust. This is an exciting opportunity to develop your knowledge and skills in specialist palliative care and be part of providing and influencing the delivery of excellent palliative and end of life care for all.

This post is a part time post of approx. 28.5 hours and is based in our community palliative care team.

The post holder's primary role will be to provide direct advanced specialist nursing assessment, support and expert advice to patients dealing with complex, distressing and life limiting illness and those important to them in the setting agreed.

Main duties of the job

We are seeking applicants that can demonstrate excellent communication skills and have a passion for delivering the highest quality holistic end of life care to patients and families.

This role will provide specialist clinical support to patients and those important to them. The role will be based predominantly in either community however rotation across the community and hospital may be considered in the future.

To contribute to the development of the palliative care service as part of the specialist palliative care team and the wider end of life agenda

To contribute to education as defined in the HDFT End of Life Education priorities.

About us

The Harrogate and Rural Dales locality has a population of approximately 160,000 people, encompasses 515 square miles and includes Harrogate and District NHS Foundation Trust, which includes Harrogate District Hospital and Ripon Community Hospital.

The Harrogate and District Specialist Palliative Care Team provides multi-disciplinary advisory support for both hospital and community patients with advanced, progressive disease and complex palliative care needs. When fully staffed, the team will be providing a face to face service 7 days a week during the day. The team work in partnership with Saint Michael's Hospice across the locality.

As well as providing specialist palliative care directly to patients and carers we are responsible, in partnership with others, for designing and delivering key quality improvements in end of life care for all across the locality. This is through education of healthcare professionals, implementation of evidence based guidelines and involvement in strategic and educational planning.

Essential

  • Registered General Nurse
  • First level degree in related subject or demonstrable experience related to clinical area
  • Evidence of post-registration education or qualification in cancer/ palliative care (minimum 20 credits) or prepared to work towards
  • Experience of caring for patients with advanced, progressive illness
  • Relevant post registration experience and consolidation at band 5 or above
  • Evidence of development in a leadership and management role

Desirable

  • Non- medical prescriber
  • Post registration experience within a specialist palliative care setting
