In line with the NHS Long Term Plan for maternity and neonatal services, we will be commencing an Enhanced Continuity of Carer role dedicated to providing enhanced care for those that require additional support throughout their pregnancy and postnatal period with the aim to reduce inequalities in maternal and neonatal outcomes.

We are seeking a highly motivated and compassionate midwife to develop the role to care for a defined vulnerability caseload of women/birthing people and their families. The post holders will be part of a shift-based continuity model where you will caseload the women and birthing people. They will continue to support the 24 hour home birth provision by contributing to the on-call roster alongside the HDFT Community Midwife team.

The post holder will be able to work flexibly and utilise their midwifery skills throughout antenatal, intrapartum and postnatal care. They will demonstrate strong leadership qualities while working closely in partnership with women/birthing people and their families and the wider multidisciplinary team to provide enhanced and personalised care.

**Please note, our Community Midwifery team operates a home-birth on call rota, and the successful candidate(s) will be expected to be on call at least once per week**

**Please note this role requires an NMC PIN. OSCE training is not available**

Main duties of the job

Care for women and birthing people with a defined health vulnerability in the community setting providing high quality antenatal and postnatal care, and intrapartum care for those choosing to birth at home.
