Job summary

hospital sooner with additional short-term support in their home environment reducing the need for inpatient rehabilitation, risk of deconditioning and interim placements
Our ARCH rotation provides the opportunity for successful applicants to rotate between core areas of the service, to develop competencies that support positive patient outcomes, 7 days a week. This provides varied opportunities across the hospital base and community.

Successful candidates will be expected to partake a 7 day service covering 8am to 8pm across all rotational areas.

Main duties of the job
To deliver rehabilitation services within a multidisciplinary team to inpatients and community patients. Working closely with other professionals to ensure positive clinical outcomes and effectiveness by providing a co-ordinated service including continuity and quality of care.
To support patient flow from hospital to their own homes by giving appropriate information, advice and education.
