Job summary

The post holder will work as a part of the Northumberland 0-19 service team, supporting the triaging and allocation of 5-19 referrals into the service. They will support a multiagency integrated working approach to children and families, ensuring referrals are directed to the most appropriate service at the earliest opportunity. The post holder will be responsible for the co-ordination and information gathering for children referred into the school nursing service for support around possible neurodevelopmental disorders. This will require a multiagency approach to information gathering and ensuring the referral is directed to the most appropriate service for the child.
Main duties of the job
To work as part of the Integrated Early Hub referral triaging team, providing in depth knowledge and understanding of a variety of health condition to support allocation to the correct team/service in a timely manner.
To contribute to the multiagency discussion and decision-making process around referral destinations, utilising the agreed multiagency criteria for involvement as guidance.
To undertake information gathering and co-ordination of neurodevelopmental referrals for the 0-19 service to ensure the referral is directed to the most appropriate agency. This will require extensive communication with other agencies such as education, mental health services and direct communication with children and families.
To work with parents and children to promote the physical, intellectual and social development of children, particularly those who are disadvantaged.
Support the completion of comprehensive health needs assessment of clients, families and communities in particular those with additional needs and contribute to EHCP.
To work in conjunction with the School Nurse to co-ordinate, plan, implement and evaluate a programme of care for the school age population and for children with specific conditions.
