North Yorkshire 0-19 service have an exciting Opportunity in the team for a new role which supports children and family to access high quality care from the best placed practitioner at the right time.
We are looking for an innovative public health practitioner with key skills in searching for health needs, and working in partnership to address health inequalities in the community.
The role requires a practitioner with extensive experience in assessing health needs, excellent knowledge of the North Yorkshire 0-19 service delivery model and of the local offer.
The successful candidate will line manage the Strategy Nurse Team, quality assure processes to ensure the lived experience of children and young people is captured and this is used to inform decision making and care delivery. The successful candidate will be required to support practitioners across the North Yorkshire Pillars to deliver interventions to meet health needs within North Yorkshire. Experience and skills in coaching & teaching will be an advantage in the role.
Main duties of the job
Management of Strategy Nurse Team including quality and performance management supervision, audit and appropriate escalations. Work collaboratively with 0-19 practitioners and multi-agency to identify individual, family and community needs and appropriate support to meet the needs.
To have oversight of all strategies coming into the North Yorkshire Service, review the rationale and delegate as required to the most appropriate person.
Confidently challenge partners when threshold of risk is not appropriate to safeguard the child/adult.
To participate in daily strategy discussions when a child is at imminent risk of harm.
To take forward any daily actions such as informing 0-19 practitioners of safety issues and to record these in the child and adult electronic record.
To liaise with other multiagency colleagues around decision making, particularly where a decision has been made to opt out of a case.
To facilitate and where appropriate provide education and support for other staff who deliver services to children and their families in the community.
