Job summary
The Advanced Clinical Practitioner (ACP) role will be part of an existing cohort of ACPs who deliver the Care of the Elderly and Frailty service for the Harrogate population under the supervision of a Care of the Elderly Consultant.
The role requires support to the other areas which form part of the Elderly Care & Frailty service e.g. our rehab ward and the virtual ward to ensure the services function as a single, seamless pathway with opportunities to share and develop knowledge and skills in both community based and inpatient services.
Main duties of the job
We are pleased to invite applications for an Advanced Clinical Practitioner post in Care of the Elderly/Frailty medicine. Our service includes a 18 bedded acute frailty ward and 2 Care of the Elderly Wards. We also provide outreach to our community based elderly rehab ward and are in the process of developing our virtual ward
We are keen to continue to expand and develop our ACP workforce to support delivery of our exciting new frailty model and applications are invited from ambitious and quality focused individuals who feel they can commit to their development and the support of the specialty. Strong clinical skills and leadership qualities are required to help drive the service forward. A willingness to contribute fully to the development of the ACP body and a keen interest in innovation and practice improvement are key.
