Job summary

Are you looking for an exciting opportunity to join the Harrogate and Rural Alliance Adult Community Services as a Diabetes Specialist Nurse? This is a rare and exciting opportunity to join forward thinking and passionate teams in delivering care and support to people living with diabetes out in the community.

The role will primarily involve supporting the community teams in the management of patients with diabetes, providing education to community based colleagues, patients/carers, supporting discharge of patients from hospital, reducing admissions, caseload reviews and service development.

Main duties of the job

Contributing significantly to the management of patients with diabetes in the community setting, facilitating a smooth transition between primary and secondary care.

You will regularly review community nurse caseloads and assess patients following discharge from hospital aiming to adjust treatments as required and reducing workload of community nursing teams

You will provide management plans, treatment advice and support for adults with diabetes in the community, their families and/or carers and act as a clinical and educational resource for other health care professionals.

You will be a good team player but able to work autonomously, always striving to improve the care of and empower people with diabetes to self-manage where appropriate.

You will provide support to community teams, GPs and community colleagues as required.

Essential

  • Registered Nurse on the appropriate part of the NMC Register
  • RGN with academic diabetes qualification
  • Non medical prescribing qualification (V300)
  • First level degree or actively studying towards
  • Post registration nursing experience at Band 6
  • At least 3 years recent clinical experience in diabetes care/acute setting/community setting

Desirable

  • Educated to or willing to work to Masters level Advanced Clinical Practice
  • Community experience
  • Research and audit experience
