Job summary
The Band 3 Fundamentals of Care (FoC) Health Care Support Worker (HCSW) will undertake the role of working alongside the FoC Team to support the wards in delivering safe, compassionated, kind and effective care. This will also include some administration responsibilities.
As a FoC HCSW you will make a valuable and important contribution to the supported delivery of high quality, safe and compassionate fundamentals of care for our patients.
Duties will include, but are not restricted to:
- To work within secondary care with the multidisciplinary team, to champion all areas of fundamentals of care such as continence, dementia, delirium, falls and enhanced care.
- To review patients on the wards as requested by the FoC Team Lead.
- To complete outstanding Lying and Standing Blood Pressures as directed by the FoC Team Lead.
- To assess and provide appropriate walking aids to patients under the direction of the FoC Team.
- Complete monthly audits on Falls, Dementia, Enhanced Care and Catheter Care.
- To assist the FoC Team in delivering planned and adhoc training.
- Some administration responsibilities for the FoC team.
These duties will be carried out under the direction, but not direct supervision, of the FoC Team.
Main duties of the job
Clinical Practice
- To accurately and promptly carry out the instructions of the Fundamentals of Care Nurse and Fundamentals of Care Lead.
- To assist in the supervision/teaching of other healthcare staff as directed.
- To be able to follow plans of care deemed appropriate by the FoC Team after the completion of FoC competencies.
- To report to the FoC Nurse any patient who gives cause for concern.
- Provide and deliver a high standard of individualised care under the direction but not direct supervision of the FoC Team.
- To accurately document, all care delivered to patients, countersigned by a registered professional, maintaining confidentiality at all times.
- To work within secondary care with the multidisciplinary team, to champion all areas of fundamentals of care such as continence, dementia, delirium, falls and enhanced care.
- To review patients on the wards as requested by the FoC Team Lead.
- To complete outstanding Lying and Standing Blood Pressures as directed by the FoC Team Lead.
- To assess and provide appropriate walking aids to patients under the direction of the FoC Team.
- To adhere to Trust infection control procedures, demonstrating an understanding of cross infection.
- To support relatives and patients demonstrating compassion and empathy under the guidance of the FoC Team
