Job summary
We are looking for a motivated, dynamic individual to join our frailty and stroke department as we continue to improve our services to provide the best care to our frail older patients with stroke disease. This is an exciting time to be joining our trust, with an expanding frailty service across several areas of the acute trust. We have close links with Leeds Teaching Hospitals and York Hospital Acute stroke services, and there is opportunity for the successful candidate to join an acute stroke on call rota at one of these neighbouring trusts or the Frailty on call rota in HDFT.
Main duties of the job
The successful candidate would be working alongside another geriatrician with a speciality interest in stroke, as well as a team of neurologists who contribute to the stroke service. We have a dedicated stroke ward with a specialist multidisciplinary team providing excellent care to our acute stroke patients, once returned from the local Hyper acute Stroke Unit at Leeds or York. We have recently recruited to our stroke clinical lead role and for the first time have two Allied Health Professionals sharing this role. We also have a neuro rehab unit, as well as close links with our community stroke team. The post holder would also be involved in providing TIA clinics and a general outpatient clinic. The in-patient cohort is a mixture of older people with stroke, and general frailty patients. We strive to ensure that all our frail older patients in hospital receive expert care, and the new post holder would provide a Comprehensive Geriatric Assessment for these patients who are on a general medicine ward. The job is based on a 10PA job plan, with 8 DCC and 1.5 core SPA, with further SPA for educational or clinical supervision. We actively encourage and support the development of our clinicians and are happy to discuss the development of sub-speciality interests and leadership and management skills.

Approximate division across the two wards of 5 patients on Granby (Stroke Frailty) and 10 patients on Oakdale (Frailty). The number of admissions to the stroke unit with the diagnosis of stroke is approximately 25-30 per month with 22-23 yearly totals at 258. TIA referrals on average total 36 per month seen Monday to Friday in clinic by stroke and neurology clinicians, approximately 2 per clinic.
There will be the opportunity to develop a special interest and run a clinic. The normal clinic ratio is 2 new patients and 3 follow ups per clinic.We would welcome visits to the hospital to meet the team see where you would be working.
