Job summary

HDFT are seeking to recruit a motivated and passionate registered Nurse to be part of the innovative 0-25 SEND Service based within Co Durham. We are dedicated to working towards improved public health outcomes for children and young people with SEND, delivering high quality universal and targeted health services and support.

Main duties of the job

The role will involve working within both school and community settings, including family homes both as part of a team and as a lone worker. The role sits predominantly within the 0 to 25 SEND Family Health team.

Key aspects of the role include: Planning and delivery of health promotion sessions and information campaigns via social media. Completion of Health Needs Assessments and Multi-agency working to support children and families with complex health and emotional health needs. Safeguarding and Looked After Health Assessments, Drop In sessions in both schools and communities, and ensuring visibility and accessibility to children, young people and families both in school and the local community.

About us

The post holder works as a member of the 0-25 SEND Family Health Team, being responsible for the assessment, planning, implementation and evaluation of a comprehensive service which meets the needs of the 0 -25 SEND population. The post holder will participate in delivering Public Health intervention and support.
