Harrogate and District NHS Foundation Trust (HDFT) is pleased to announce an exciting job opportunity within the new frailty 'Hospital at Home' service.
The team are looking for a specialty doctor or GP who is enthusiastic about providing excellent, holistic care for older people living with frailty. The post holder will be based at Harrogate District Hospital, but will primarily be working within our new and developing frailty virtual ward (Hospital at Home) service.
We have recently been part of the national Acute Frailty Network programme and this is an exciting time to join the HDFT frailty team as we continue to strengthen and develop our services, with the aim of providing exceptional care for our local population living with frailty. The successful applicant will be encouraged to actively contribute to service development within the department.
This is a full time position but applicants looking for less than full time employment are encouraged to apply.
Main duties of the job
Since January 2023 HDFT has been providing a 'Hospital at Home' service for older people living with frailty. We are able to accept patients on to the caseload 'stepped down' from an acute ward or the emergency department or 'stepped up' directly from the community. At present, 'step up' referrals are taken from the ARCH team or Urgent Care Response (UCR) team.
Care at home is provided by the consultant of the day, a specialist frailty ACP and other members of the ARCH team. Which team member(s) undertake each visit is determined by complexity and need. The post-holder is expected to work closely with and provide supervision to the ACP workforce. The post holder is expected to make home visits and must therefore be a car driver with their own transport.
The successful candidate will be expected to manage patients with a range of common acute frailty syndromes and make a holistic assessment of patients on the case load using the principles of comprehensive geriatric assessment. The ability to communicate effectively within the team and when liaising with other specialties is essential. A daily MDT meeting is held to discuss all patients on the caseload.

Clinical responsibility with consultant support for elderly patients with frailty on the Virtual Frailty Ward and the Acute Frailty ward including a daily MDT. Responsibility for supporting the community virtual ward and ARCHES team. This work is in partnership with Dr Watt, Dr Ralston and Dr Fishlock, Dr Norman and Dr Atkinson and includes prospective cover.
To be involved in the development and governance of the Virtual Frailty Ward and Acute Frailty Services
To regularly devote time for the training and supervision of junior medical staff and/or ACP trainees, and contributing as appropriate to undergraduate, postgraduate and continuing medical education.
To manage personal workload in such a way as to ensure that capacity is maximised and quality of care is optimised.
Provide clinical leadership and supervision to the nursing and support staff within the team.
Provision of advice to Trust Managers, local General Practitioners and other interested parties, on the provision, planning and development of medical services.
Explore service modernisation opportunities such as preventative care and development of community led initiatives/support to primary care.
To participate in the recruitment of junior medical staff and staff of other disciplines as appropriate. To assist the Human Resources Department to follow the local and national employment policies and procedures.
To participate in audit with the support of the Trust Clinical Governance department
