Job summary

We are inviting applicants for a new and exciting opportunity as a Patient Pathway Manager within one of our operational directorates at Harrogate & District NHS Foundation Trust.

We are looking for a passionate individual who can help us to deliver an efficient, high quality medical records service across the Trust. This role is essential to providing safe care to our patients, ensuring that medical records are in the right place at the right time for patients to be treated and then returned to the correct storage location.

This post will work across our Long Term & Unscheduled Care Directorate to ensure processes and systems are in place to support robust management of each patient pathway from the point of referral to discharge.

This role is a senior member of the team, providing leadership and guidance to our patient pathway teams. The role will be key to helping us achieve our outpatient targets.

The post will involve day-to-day interface with clinicians & managers across the Directorate and wider organisation, to direct and change perspectives on patient pathways at all levels, whilst promoting the use of high-quality information to inform effective decision-making.

Main duties of the job

The post requires an efficient, caring and proactive individual with an appreciation and understanding of the confidential nature of the work.

Applicants should be able to demonstrate accuracy and attention to detail, be an excellent communicator with good telephone skills and have the ability to meet deadlines on a daily basis whilst maintaining a positive approach, and providing a quality service that supports our patient care.

This role requires an individual with a good understanding of outpatient access, information governance and waiting list management and the ability to understand, validate and present data.

Central to the role will be working with the direction of the Operational Director to ensure targets are appropriately monitored and managed including working to identify underperformance, risk to future performance and assisting Operational Managers in tracking remedial action to improve.

The person in this role will be required to develop and assume a good understanding of the Trust business requirements.

A fundamental part of the role is to use data, which will be used to support service developments/improvements.

A flexible, organised and focused approach to work tasks is essential.
