The York Discharge Liaison command centre is looking to recruit an enthusiastic and self-motivated Discharge Liaison Specialist Nurse/Occupational Therapist/Physiotherapist. The hub provides a service to patients and customers across York, Selby, Ryedale, East Riding and North Yorkshire.
Please note that for this post we are welcoming applications from both registered Occupational Therapists, Physiotherapists, and Nursing Professionals.
Main duties of the job
You will join a developing inter-agency team, working closely with, ward staff, Social Care, Continuing Health Care, community rehab units, intermediate care and the voluntary sector. You must be able to demonstrate collaborative working, as well as being able to build effective relationships with colleagues from other healthcare professions.
You will be required to have sensitive discussions with patients and their families, and on occasion be able to diplomatically challenge conversations, in order to achieve the best outcome for patients for a safe discharge.
You will benefit from working within a small, friendly, experienced, pro-active team who are looking for a colleague with a positive approach and has a flexible and patient centred approach.
What are we looking for:
You should be a dynamic, forward-thinking practitioner, with the desire to develop and improve services. You must have relevant clinical experience, and excellent communication and organisational skills. The successful candidate will hold their own caseload within the acute hospital.
The ability to escalate concerns, via appropriate channels of escalation, is also essential in this role.
The successful candidate will be required to use a variety of computer applications and should be confident in IT skills.
