GI Cancer Care Coordinator

Job summary

We are seeking a dynamic and motivated GI Cancer Care Coordinator to join our supportive Cancer Nursing Team at York and Scarborough Teaching Hospitals. Our team is committed to delivering excellent care and improving the experience of people affected by GI cancers, and your role will make a meaningful difference.

We are looking for a proactive, compassionate individual who can work independently and as part of a busy multidisciplinary team. You will support specialist nurses in providing safe, effective, high-quality care.

The ideal candidate will have strong communication and interpersonal skills and feel confident working with clinicians, patients, and allied professionals. You will demonstrate empathy and provide clear, reassuring information to patients and families, often in emotive situations. Excellent organisational skills, attention to detail, and confidence using a range of IT systems are essential, along with the ability to manage confidential information appropriately. You should be self-motivated and committed to ongoing personal and professional development.

This is a full-time role (37.5 hours per week) based primarily at York Hospital.

Main duties of the job

Act as a key contact for cancer patients and families, responding with professionalism, empathy and sensitivity. Provide consistent, person-centred support across diagnosis, treatment, follow-up, and where appropriate palliative and Best Supportive Care pathways, ensuring a positive experience during complex and distressing times. Offer clear information, guidance and emotional support, escalating concerns to the Clinical Nurse Specialist (CNS) as required.

Undertake Holistic Needs Assessments (HNAs) identifying physical, emotional, social and practical needs. Liaise with relevant professionals to address concerns, document outcomes, contribute to treatment summaries, and arrange timely referrals and signposting to appropriate services.

Make proactive support calls, including for patients on palliative or BSC pathways, recognising changes in wellbeing and escalating concerns appropriately. Manage correspondence effectively and assist with practical support such as transport and non-clinical referrals, including benefits and community services. Maintain up-to-date knowledge of local support networks.

Monitor cancer tracking systems to ensure pathway coordination and accurate documentation, including patient interactions, reports, and referrals. Support the CNS team with administrative tasks, maintain patient information resources, and coordinate handovers to ensure continuity of care. Contribute to service improvement, health promotion, and ongoing professional development.
