York and Scarborough Teaching Hospitals NHS Foundation Trust provides adult community services as part of an integrated acute and community care Trust, working closely with partners across health and social care on behalf of our communities across the Vale of York.

We are looking for a highly motivated Project and ImprovementManager to join our Community Services Team, to support the delivery of our ambitious Transformation programme. This is a 18 month post based in York (with a requirement to travel across all Community sites).

Main duties of the job

Our successful candidate will join an established friendly Community Team and will work closely with the Community Services Clinical and Operational Teams.

You will be responsible for the overall management, qualityimprovement and delivery of specific areas within the Transformationprogramme of work for example implementing Neighbourhood Working with external partners, Inpatient Unit Transformation and Discharge and Admission Avoidance.

You will have experience of service development and project management, managing and delivering change effectively on complex projects. They should be motivated to function independently, using initiative to manage their own workload without close supervision to work to agreed deadlines. You will also motivate, negotiate and influence on complex and sensitive projects with others, at a strategic level.
