Healthcare Support Worker - Transfer Team Associate

Job summary

The Transfer Team Healthcare Support Worker (HCSW) will assist in preparing and transferring patients from the Emergency Department to the acute admission wards, from the acute admission wards to downstream wards, and from all areas of the hospital to the Patients Discharge lounge.

Transferring patients from one clinical area to another on either in a bed or a chair whilst working closely with members of the multi-disciplinary team within the clinical areas and management teams.The Transfer Team HCSW will ensure patient safety is maintained and patient experience is positive by treating all patients, relatives and colleagues with respect, dignity, and courtesy in accordance with Trust values. Ensuring at all times you act within your sphere of competence.

Main duties of the job

  • Escort patients safely to other departments within the hospital and be able to give a summary of care to the nurse in charge of the patient.
  • Assist in preparing patients for transfer which includes personal hygiene, continence care and packing up belongings in conjunction with ward / department staff.
  • Transfer patients with oxygen requirements following an assessment from registered nurses who will be responsible for disconnecting and reconnecting (administering) oxygen at both ends of the transfer.
  • Transfer patient medication as required under the direction of ward staff who are responsible for checking the medication at both ends of the transfer.
  • Provide appropriate equipment and aids as delegated by clinical staff, or in accordance with standard operating procedures and protocols.
  • Listens to patients concerns, acts appropriately to patient requests, documenting actions and passes information or issues that they are unable to deal with to the nurse in charge.
  • To be flexible and be able support any ward or department when required within the role of an HCSW.

You must be able to commit to our Full Time HCSW Academy for the first 4 weeks of your employment with our Trust. Please note our HCSW Academy takes place at our training site in Holgate, York. We are unable to provide travel options or reimburse travel costs for attendance at this.
