Job summary
We are looking for an experienced heart failure specialist nurse with NMP qualifications to join our team based at Clementhorpe Health Centre offering services across the Vale of York at several sites.
Main duties of the job
Assessment and management of heart failure patients initiating an evidenced based management plan according to individual need. Will be expected to work across all services within the team which incorporate acute assessment and delivery of IV diuretics, community clinic services and home visiting where appropriate.
NMP qualification is essential, advanced clinical assessment skills and appropriate knowledge and experience of managing heart failure
Work collaboratively with colleagues across the acute, community and primary care networks.
Develop new and exciting ways of managing this complex group of patients and leading in research and innovation
The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy. Access to a vehicle that can be used for business purposes and hold a full and valid driving licence (role will involve travel to numerous addresses that cannot be easily accessed by public transport means) is required.
