Job summary

This exciting opportunity is for a Band 6 Physiotherapist to join our Cardiac Rehabilitation Team at York and Scarborough NHS Foundation Trust Community Services.

This is a one-year, fixed term post which can be a secondment opportunity, to deliver Specialist,comprehensive Rehabilitation services to a cohort of complex cardiac clients in a group exercise class delivery setting alongside a Band 4 assistant practitioner and the Cardiac Rehab Specialist Nurse Team.

Training will be provided to assist in building your skills for the delivery of holistic long term condition Rehabilitation.

The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy

Main duties of the job

This post would suit a Band 5/6 looking to progress their clinical expertise and holistic rehabilitation skills. We have an established support network so all applicants who are unsure about their readiness for a post of this nature are also encouraged to apply.

You will be part of a skilled multi professional team, providing a high-quality Rehabilitation service. You will be involved in all aspects of the assessment, treatment, and discharge process, and work alongside both health and social care colleagues to ensure that patients remain as independent as possible and achieve the best possible health and social outcomes.

You will be expected to undertake a comprehensive assessment of patients, including those with diverse or complex/multi pathology presentations. Use advanced clinical reasoning skills and assessment techniques and provide and deliver an effective treatment plan and to evaluate any intervention.

You will have access to managerial, clinical supervision and appraisal opportunities to identify objectives to progress within the four pillars of practice.

The role involves triage, pre and post assessment in line with National guidelines to Cardiac Rehab patients including those post ischaemic cardiac events and cardiac surgery patients alongside group exercise class delivery and provision of a safe effective home exercise programme.The current B7 Specialist Physiotherapist in Cardiac Rehabilitation will offer clinical support.
