Job summary

An exciting opportunity has arisen within the Hospital at Home team.

The post holder will be providing high quality nursing care to the patient within their home environment.

Duties will include assessing patients in the hospital in preparation for them returning to their home setting. This can be in their own home, a rehab facility or a residential/nursing home.

They will then prepare documentation and applicable equipment in preparation for the patient's timely discharge from the acute care setting.

The post holder will actively monitor the patient whilst they are under the Hospital at Home service, including bloods and observations. They will be actively managing the planned and emergency reviews of the patient during the patients stay on the Hospital at Home virtual ward.

The post holder will also be proactive in the audit process, to monitor standards throughout the service in conjunction with the Team Manager and Matron.

They will be directly involved in managing the patients on the waiting list for the service and assessing patient need and priority. This will involve sign posting patients to different services to provide their nursing care.

Main duties of the job

1. To be responsible for the assessment of care needs, delivery, implementation and evaluation of management plans for individual patients using a problem-solving approach and evidence-based care.

2. To work with colleagues, or independently, to provide the highest possible standard of care for patients assessed as safe and medically suitable to remain at home or continue care at home.

3. To work with specific groups of patients within agreed pathways in Primary and Secondary Care and taking responsibility for their ongoing management and safety.
