Job summary
Under the direction of the Lead Midwife for Patient Safety the post holder will be responsible for the provision of practical support, expertise and facilitation to the Service Line, across a wide range of patient safety and quality improvement activities.
The post holder will ensure that the Service Line is appropriately supported in order to deliver their Quality Governance agenda in line with the Patient Safety Incident Response Framework (PSIRF). This will mean provision of support relating to the management and investigation of Patient Safety Incident Investigations (PSIIs), incident reporting, risk registers, clinical audit plans and clinical effectiveness activity. The post-holder will also work closely with the Quality Governance teams (patient safety, audit & effectiveness etc.) and provide a 'link' between the Trust Management Directorate and the Service Line
The post holder will ensure provision of support to the Service Line for governance meetings and will assist the Lead Midwife for Patient Safety and other key personnel in the Service Line in the implementation of the Quality Governance Framework, dashboards and reports
Main duties of the job
Report to the Lead Midwife for Patient Safety, with the oversight of the Head of Governance and Quality Improvement, Heads of Midwifery, Operational Managers and Director of Midwifery, any issues that might affect the provision of care or service within the Service Line being supported.
Support the maintenance of the Service Line risk registers in liaison with the Lead Midwife for Patient Safety, Head of Midwifery, Operational Managers, Director of Midwifery and the Patient Safety team.
Ensure all incident reports involving or affecting Women's Services are actioned as appropriate in accordance with the PSIRF.
Identify and communicate high scoring risk issues and incident trends within the Specialty. Produce monthly reports on incident reporting to support Specialty review of trends and facilitate analysis of all incidents and to communicate and share Trust wide lessons learnt and practice changes as required.
Ensure that all incidents and near misses are reported by staff within the area of responsibility in accordance with the Trust policy and support the investigation of PSIIs, ensuring appropriate action plans are developed and monitored, and lessons are learned and disseminated.
