Haematology Clinical Nurse Specialist

Job summary

An exciting opportunity has arisen within the Haematology Clinical Nurse Specialist team within the Haematology at Leighton service. We are looking for a highly motivated and enthusiastic registered nurse with at least 4 years relevant experience to join the Christie at Leighton Haematology service. The post holder will provide specialist nursing advice and support to patients and families and will coordinate patient care and act as a 'key worker' for a nurse-led patient service. The post holder will be working in conjunction with other haematology nurse specialists, advanced clinical practitioners, Consultants and a wide range of administrative staff.

Main duties of the job

Provide specialist nursing support in haematology clinics and holistic needs assessments.

Provide nurse led clinics including patient stratified follow-up

Work with the haematology nursing team and be a 'key worker' for patients and their families

The management of complex treatment pathways

Expert resource for patients and colleagues

DUTIES AND RESPONSIBILITIES

  • Provide evidence based individualised patient care according to patients changing healthcare needs, through electronic holistic needs assessment.
  • Participate in the clinical management of patients and formulate clinical management plans as per medical instruction and patient agreement.
  • Act as a `Keyworker` for a clinical case load and facilitate the co-ordination of patient care.
  • Support the haematology team to assess patient`s information needs and provide relevant information to meet those needs, ensuring the focus is on the promotion of health as well as illness.
  • Accept and make appropriate referrals to multi- professional disciplinary team and take action if clinically indicated.
  • Communicate complex, sensitive, or contentious information to patients and carers, which may include results and management options. Ensure patients receive appropriate information and support in relation to their illness and treatment.
  • Contribute to the collection of quantitative and qualitative data, utilising this data by providing written reports when required which provide evidence of productivity, outcomes, and quality.
  • Actively integrate research, theory, and practice by undertaking surveys or audits, as necessary.
  • Participate in the development of trust policy and clinical guidelines within haematology.
  • Practice evidence-based healthcare and be responsible for developing clinical policies within the Christie @ Leighton Service
  • Assess patient`s adverse events to therapy to enable appropriate management and symptom control.
  • Develop skills in advanced communication for all aspects of patient assessment, care, and support.
  • Provide a telephone clinic for MPN/MGUS/CLL & electronic HNA.
