Job summary
The Critical Care Outreach Team is a proactive service that provides early recognition, assessment and management of the deteriorating patient. We are looking for compassionate, enthusiastic driven practitioner who is passionate about making a difference in the care of the acutely unwell and deteriorating patient.
You will provide autonomous specialist advice to support and facilitate high quality individualised care to the acutely ill and deteriorating patient in collaboration with the multidisciplinary team. You will assist in facilitating timely admission to critical care and support the discharge from critical care involving appropriate members of the multidisciplinary team in the decision-making process.
The successful candidate will be able to make informed decisions within their own area and scope of professional practice to ensure agreed standards, objectives and targets are met and become an expert practitioner within the field of critical care and acute oncology. Additionally, this role offers support to both our nursing and medical colleagues with acutely ill or deteriorating patients and be a member of the resuscitation team. The successful candidate will also be an integral member of the Critical Care Outreach educational faculty.
Main duties of the job
The practitioner will work as part of the critical care outreach service. They will provide expert clinical practice, care, and intervention for the acutely ill, deteriorating and critically unwell ward patient across the trust on a 24-hour shift rotation that covers 365 days per year.
The practitioner will perform patient assessments, undertake appropriate investigations, and initiate, evaluate, and modify a range of interventions to prevent patient deterioration. They will assist and facilitate the timely admission and discharge of patients to and from the critical care area by early care intervention and supportive of their continuing recovery within the ward environment.

- Communicate complex clinical and supporting information/findings to patients and significant others according to individual situations.
- Liaise with the multi-disciplinary team with respect to the management of the acutely unwell patient.

- Coordinate and plan management of patients requiring clinical interventions throughout the Trust.
