Job summary
The Urgent Treatment Centre at Mid Cheshire Hospitals NHS Foundation Trust and are looking for an Advanced Practitioner to join our small dynamic and friendly team making a real difference in patient care. The team is a GP led service, made up of, ACPs, senior nurses, technician's and admin support staff.
The Advanced Care Practitioner will need to be a competent in using their expert clinical knowledge and skills. They must have a full masters and prescribing rights. They will have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of adults and children.
The role is offered either working short shifts (6 hours: 09:00-15:00 and 15:00-21:00) or longer shifts (11.5 hours: 09:00-21:00).
Main duties of the job
The post holder will require excellent communication skills which should include communicating clearly, sensitively and effectively with patients, families and professionals.
We are looking for someone with high energy levels, a positive can do attitude that can work alongside colleagues to deliver safe, kind and effective care.
Work autonomously, carrying out in-depth clinical assessment of patients with complex multiple healthcare needs. Being able to diagnosis, treatment and discharge as appropriate.
Work collaboratively, maintaining effective communication, with other professionals and agencies to ensure patient needs are met.
Demonstrate advanced communication skills and provide support when delivering diagnosis.
Collect, collate, evaluate and report information, maintaining accurate patient records.
Participate in the development of services and assists the implementation of change
Provide education and development opportunities for others. Being able to support all members of the UCC team
Develop own specialist clinical knowledge and participate in the development of others' clinical knowledge.
