Job summary
We have an exciting opportunity for an experienced registered nurse or AHP, with a broad set of clinical skills and the ability to provide effective operational leadership andmanagement to the virtual ward team. This role will require you to have overall responsibility for the caseloads within Virtual Ward to ensure that high quality, efficient and safe care is provided.
You will report into the Home First Community Service Manager who will support and guide you through regular 1-1 meetings and coaching of continuous improvement methods. This role will also give you experience of working with our partners within the wider Integrated Care System.
We are looking for an enthusiastic, flexible leader who possesses the clinical, leadership and digital skills to promote quality improvement for alocal population. The Virtual Ward Service covers 7days a week 8am-8pm
Main duties of the job
To provide clinical and operational leadership and transformation support within the developing Virtual ward service.
Leading and liaising with the clinical and non-clinical teams involved in the various services/Projects.
Producing /collating ongoing information to demonstrate service development achievements through data analysis via BI Dashboards.
Work in partnership with commissioning and provider services across Cheshire.
Support development and production of evaluation reports.
Contributing to the management of allocated Budget and other associated resources.
