Job summary
The Cardiac Rehabilitation Service at Leighton Hospital is looking for a Band 7 Clinical Nurse Specialist or Allied Health Professional, to lead the development, implementation and evaluation of seamless specialist service, ensuring patients receive the highest standards of clinical care. The post holder will demonstrate a high level of expertise within the specialist service, providing advice, education and support to staff, patients, their families and carers. The post holder will actively initiate and participate in the clinical activity and audits within their specialist area.
Critical to the role will be the ability to liaise with our clinical colleagues across MCHFT, Primary Care and external centres to promote a cohesive cardiac rehabilitation pathway from discharge planning to long term maintainence of health and wellbeing
Main duties of the job
Lead a specialist practice, providing expert professional advice within the agreed guidelines and protocols, including medical colleagues and other professional groups.
Ensure their clinical and managerial skills and those of their team are appropriately deployed to provide a comprehensive rehabilitation package, for patients who have had admission for Acute Coronary Syndrome, revascularisation procedures, and/or following valve surgery.
Deliver a clear, knowledgeable and practical approach to service delivery that reflects the BACPR rehabilitation standards, promoting a patient-centred menu driven programme.
Co-ordinate and develop the rehabilitation team so that services both at Leighton Hospital and at our off site locations deliver the education and exercise programmes to support patient recovery.
Liaise with our clinical colleagues at Leighton, Primary Care, and external centres to promote a cohesive cardiac rehabilitation pathway from discharge planning to long term maintenance of health and well-being.
Facilitate the collection and data entry for the National Audit of Cardiac Rehabilitation, supporting data submission and reporting.
Use the relevant electronic systems to promote excellent communication skills across the secondary/primary care interface.
Promote evidence based practice and develop the team in response to innovations and changes in service delivery.
Create a learning environment for staff development and student supervision, of a variety of disciplines.
