Job summary
We are looking to recruit an experienced and competent healthcare professional to provide continuous responsibility for the leadership and management of the Children and Young Peoples Home Care Team.
The Children and Young Peoples Home Care Team consists of qualified paediatric nurses who specialise in caring for children and young people with a specific nursing need with care being delivered across integrated pathways of services, primary, secondary and tertiary care, transition into adult services, health, children's social care, and education.
The post holder is responsible for achieving clinical excellence in the quality of care provided through the best use of the team, by allocating work appropriately, using resources effectively, developing and implementing policy and guidelines, and by controlling an agreed financial budget. The post holder will work autonomously, collaboratively and co-operatively with others to meet the needs of patients and their families and take an active part in contributing to the wider activities and strategic plans of the Trust.
Main duties of the job

The post holder is required to be a proficient health care professional who can provide expert evidence-based practice and clinical expertise in a specialist area in order to:
- provide specialist nursing care for children and young people in the community, to reduce the length of hospital admissions / attendances where it is safe to do so.
- To provide an effective liaison link between hospital and home, encouraging a unified approach prior to discharge home.
