Job summary

Central Cheshire Integrated Care Partnership (CCICP) have an exciting opportunity to join our dynamic and forward-thinking IV at Home Team as a Team Leader/Clinical lead for the service expansion into East Cheshire NHS Trust.

The post holder will be required to lead our clinical IV Team to cover the footprint of East Cheshire delivering IV antibiotics to patients in the community setting. The Team Leader will work closely with Hospital Consultants, Microbiology and Pharmacy colleagues ensuring an MDT approach to patient care.

The post holder will require advanced communication skills and demonstrate quality improvement experience that will help to deliver the service expansion within the East footprint.

Main duties of the job

We are looking for a highly skilled Nurse who possess experience of working with patients who are being treated with Intravenous antibiotics within the community setting. You will have a broad set of clinical skills and the ability to provide supportive leadership and operational management to the IV at Home Team. This role will require you to have overall responsibility for the caseload within IV at Home Team and provide advance clinical leadership in providing safe and effective care.

You will report into the CCICP Operational Manager who will support and guide you through regular 1-1 meetings. This role will also give you experience of working with our partners within the wider Integrated Care System.

SERVICE DELIVERY

  1. To lead others from a base of clinical credibility in order to model and facilitate improved practice and partnerships.
  2. To demonstrate positive leadership skills identifying and prioritising the workload to ensure a safe operational system, referring to Community Operational Manager as necessary.
  3. To make a proactive contribution to the future development of the OPAT service with CCICP, Cheshire East and Cheshire and Merseyside
  4. To support the Community Operational Manager in the effective management of financial budgets.
  5. To respond promptly to any incident or risk relating to patients, self or staff within the team which occur whilst on duty and ensure the timely and accurate reporting of the incident/risk in line with Trust policy. This includes the investigation of all incidents and formulation of action plans to ensure that identified issues/risks are addressed.
  6. To monitor and manage team members sickness in line with Trust absence and wellbeing management policy, and record on the recognised Trust system.

  15. Ensure that all team members have opportunity to receive clinical supervision and to provide regular mentorship and coaching to staff across the community.
  16. Assess, plan and implement a programme of care to meet the patients physical, social, emotional and environmental needs evaluating care at regular intervals, encompassing a rehabilitative/self-care approach where necessary.
  17. To facilitate attendance and representation at multi-disciplinary meetings in order to work in collaboration with the wider team including the consultant, pharmacist and microbiologist.
