Job summary

This is an exciting opportunity for an experienced band 5 to join a 12 month pilot project to deliver care closer to home for patients undergoing treatment for haematology/oncology conditions.

The post holder will carry responsibility and accountability for the health care management of their patient group, in collaboration with senior nurses and Team Leaders. The band 5 nurse will be responsible for assessing, developing, implementing and evaluating family centred programmes of care in line with the project.

Main duties of the job

The successful candidate will be part of a team delivering care closer to home for haematology/oncology patients. This will include chemotherapy and supportive care treatments.

You must be able to work well as part of a team or alone using initiative whilst also recognising limitations.

Excellent communication skills with service users and other service providers is essential to co-ordinate effective care delivery.

CLINICAL RESPONSIBLITIES

1.1 Maintain high standards of nursing care and promote an environment suitable for the care of children, ensuring that all planned carefully reflects latest research developments.

1.2 To assess, develop, implement and evaluate family centred programmes of care in partnership with colleagues or senior staff and ultimately without direct supervision.

1.3 Actively participate in mentorship for junior staff and students and participate in clinical supervision.

1.4 Ensure child protection issues are acted upon promptly and appropriately.

1.5 Continuously develop the clinical role in order to be able to work independently, anticipate care needs of patients and actively manage their journey through the Trust. This includes being able to work independently or as part of a team in potentially stressful, distressing or emotional circumstances including life support and assessment

1.6 To extend and expand nursing roles within the nurses competence, and with appropriate training as guided by local and departmental policies and procedures.

MANAGEMENT

2.1 Aim to promote high moral within the nursing team by good staff relationships and clear lines of communication.

2.2 Monitor effective use of resources, in collaboration with colleagues.

3.2 Enter appropriate data into the EPR ( Meditech expanse ) in a timely manner and ensure all nurse led screens are completed

3.3 Participate in relevant departmental meetings and represent the department at Trust wide meetings when appropriate, disseminating information as appropriate.

3.4 Act in a way to promote confidentiality, dignity and trust of patients, parents, carers and other staff members.

3.5 To ensure accurate and confidential documentation is maintained in accordance with the NMC, Trust and departmental guidelines.

TRAINING/EDUCATION/RESEARCH

4.1 To support the induction of new members of staff with a period of preceptorship and to participate in the mentorship of students and to supervise and teach others.

4.2Update personal knowledge and skills by attending and participating in study days, conferences and in-service training, including mandatory training sessionsto comply with trust and departmental requirements. Achieve personal training needs identified through a yearly personal professional review with your manager.

4.3 Participate in clinical supervision

4.4 Identify areas for research and contribute and participate in nursing and medical research programmes.

4.5 Disseminate information gained from research.

4.6 Participate in quality monitoring in conjunction with Trusts Clinical audit and department audits.
