Job summary
An exciting opportunity has arisen for a talented, visionary individual with widespread experience in the field of Patient Safety. We are looking for an experienced and innovative leader who will support the Patient Safety Specialist and wider Patient Safety Leadership Team in their ambitions to implement the Alder Hey Patient Safety Strategy and PSIRF. The postholder will have proven leadership skills, experience of working at a strategic level, be highly visible, and be able to motivate and engage individuals and teams across the organisation. With demonstrable and extensive investigation and learning response expertise, the successful applicant will hold a well-developed skillset to help advance the delivery of the Trust-wide Patient Safety Programme.
Main duties of the job
The post holder will :
- be expert in providing robust and evidence-based learning reviews utilising agreed frameworks which support achievable patient safety improvements and learning.
- work to achieve the Trust's objective of a psychologically safe learning environment using restorative, just and learning principles.
- engage with patients, families, carers and significant others to ensure their voices are heard and findings and learning/ actions are agreed prior to completion.
- work collaboratively with subject experts to ensure efficacy of reviews.
- ensure all staff involved in the incident are given the opportunity to participate, review drafts and agree the final learning review prior to quality assurance processes and final sign off by Executive leads.
- work to the patient safety PSII and learning review criteria, standards and timescales set out within the Patient Safety Incident Response Framework.
- signpost those who need further support post incident appropriately including families, carers, significant others and staff.
- escalate any emerging and urgent clinical risk appropriately.
- extrapolate and escalate any conduct or professional concern directly with the Patient safety leads at the earliest opportunity.
- attend and be available, as is reasonable, to present the findings of reviews at internal and external forums as requested.
- ensure their work aligns to the relevant polices within the Trust.
