Job summary

Cardiac rehabilitation (CR) is a Program for all patients who have had a cardiovascular event and are eligible to participate in a program.

The evidence suggests that ongoing attendance at cardiac rehabilitation programmes, is cost effective and improves clinical outcomes and also a reduction in unplanned re-admissions. Starting cardiac rehabilitation as soon as possible after a Cardiovascular event can improve recovery.

Cardiac rehabilitation is led by a clinical lead with a full Multi-disciplinary approach, following the British Association of Cardiac Rehabilitation and Prevention (BACPR) there is a mixture of secondary and primary CR programmes across Cheshire and Merseyside network . Liverpool heart and chest covers a wider geographical footprint.

The Trust Cardiac Rehabilitation Service facilitates a seamless pathway into the cardiac rehabilitation programme.

The Cardiac Rehabilitation Assistant will support the Service Lead to deliver face to face comprehensive lifestyle advice and will be a visible ward presence supporting ward staff to generate a quality referral. The role includes monitoring of referrals, managing and prioritising phone calls and additional administrative tasks to support the smooth running of the service.

Main duties of the job

  • Deliver high quality, compassionate care under the direction of a Registered Nurse with a focus on promoting health and independence.
  • Have attitudes and behaviours compatible with NHS Values.
  • Work as part of a clinical team delivering care that focuses on the direct needs of the individual. This will include working autonomously and collaboratively with members of the wider MDT.
  • Carry out specifically delegated clinical care tasks and responsibilities to a high standard of competence, under the direction and supervision of a registered nurse.
  • To support the delivery of high-quality phase 1 Cardiac rehabilitation for hospital inpatients and support ward staff to make timely, confidential, and efficient referrals to community stakeholders across a wide geographical footprint.
  • The post holder will assist the CR team leader providing professional and clinical support to patient eligible for Cardiac Rehabilitation.
  • The post holder will ensure that a high standard of patient care is provided in line with, Nice Guidelines and BACPR standards framework.
  • Responsible for the day-to-day management of admin processes, such as filing referrals, answering patient/carer and stakeholder calls.
  • Inputting and keeping up to date benchmarking data on Microsoft XL spreadsheets
  • Sign post patients appropriately to wider services including smoking cessation, alcohol services, anxiety management and exercise referral schemes.

  • Communicate and liaise with ward staff to notify them of missed/incomplete referrals, escalate any concerns to Lead cardiac rehabilitation nurse or Inphase if needed
  • Answer any phone queries from staff/patients/stakeholders and act upon them promptly referring any issues to Cardiac rehab nurse
  • Act upon any Email correspondence from external stakeholders and file Electronically
  • Complete incident reporting (Inphase) on advice from CR nurse and file Electronically
  • Access patients records within EPR to resolve any stakeholder queries, document information clearly in the records
