Job summary

We are looking for an enthusiastic nurse with experience in genetics and/or cardiovascular disease, especially working with Familial Hypercholesterolemia (FH) patients, to deliver FH service across Cheshire and Merseyside.

They will work in partnership with the Consultant Clinical, Lead FH Nurse, Innovation Agency and other key stakeholders and will be expected to demonstrate professional initiative, independence and autonomy in the development of FH services across the locality. They will be part of a team developing and implementing a family cascade, audit and diagnostic service for the identification and management of individuals with FH and have responsibility for delivering clinics, counselling and organising biochemical and genetic tests, and provision of treatment.

They will be expected to manage and deliver expert clinical practice, providing specialist advice and support to those patients and families with suspected FH

There will be an expectation that they will work with primary care staff, consultant physicians, consultant nurse, nurse specialists and other related professionals to develop high levels of care for patients with FH.

Main duties of the job

The post holder will work with the multidisciplinary team to deliver high quality and efficient care for patients, demonstrating professional initiative, independence and autonomy in the development and maintenance of FH services across the locality.

They will undertake diagnostic testing for FH using genetic and biochemical tests. This will involve discussing complex information in order to inform and counsel individuals on the significance of such investigations both for the individual and their family members.

Either within multidisciplinary clinic or/and nurse led clinic the nurse will undertake clinical physical patients' assessment, provide further specialist advice and education re treatment options, prescribe medication as appropriate.

Work with primary care staff, consultant physicians, nurse specialists and other related professionals to develop high levels of care for patients with FH.

To provide professional support, clinical expertise and specialist knowledge and training to all those professionals involved in providing care to patients with FH.

PRINCIPAL ACCOUNTABILITIES

CLINICAL

1. Deliver high quality and efficient care for patients in conjunction with the multidisciplinary team. Provide direct and indirect patient care to a specific group of patients.

  1. To undertake diagnostic testing for FH using genetic and biochemical tests. This will involve discussing complex information in order to inform and counsel individuals on the significance of such investigations both for the individual and their family members.
  1. Provide information, education and support for patients in the diagnosis and follow-up of their condition.
  1. Either within multidisciplinary clinic or/and nurse led clinic the nurse will undertake clinical physical patients assessment, provide further specialist advice and education re treatment options, prescribe medication as appropriate.
  1. To work with the multidisciplinary team in developing the use of new and emerging therapies for patients with FH.
  1. Cannulate and undertake blood sampling from the specific group of patients.
  1. To act as an advocate for patients and families, acting in the patients best interests at all times.
  1. To use the FH project software to record information for family cascade testing, as directed by the project manager.
  1. Liaise with primary services including GPs, community nurses, and health visitors to improve communications.
  1. To work with the multidisciplinary team to ensure the genetic database for patients is kept current and updated according to protocols.
  1. Deal with complex situations as they arise. Able to analyse and make decisions in relation to patient risk and care.
  1. Provide specialist clinical advice within the multidisciplinary team

1. Work alongside local and national networks and partnerships promoting and disseminating evidence of service development.

  1. Work in partnership across the primary and secondary sectors to identify ways of improving services using integrated care pathways and educational packages as appropriate.
  1. Demonstrate the ability to maintain effective relations and communication networks with internal and external agencies.

  1. Initiation and implementation of change where necessary, including supporting primary care with running searches and making appropriate referrals.
