Job summary
The Targeted Lung Health Check (TLHC) programme will enable the early detection of lung cancer in order to save lives. Cheshire and Merseyside has been identified as one of the higher areas of disease prevalence, in which significant impact can be made from the recognition of early stage lung cancer and offering curative treatment.
As a Target Lung Health Check Nurse Associate/ Assistant Practitioner, you will actively participate in undertaking lung health checks for the eligible population, as well as providing health education and smoking cessation.
The post holder will be expected to effectively communicate with members of the public from a variety of different backgrounds and be able to rely on your organisation skills to meet the requirements of our patients, and to work collaboratively and cooperatively with the nursing and wider multi-disciplinary team.
Main duties of the job
Work collaboratively with other health professionals to ensure that patients' needs are met
Communicate effectively with patients, providing reassurance and support
Communicate effectively within a multidisciplinary team and key stakeholders
Ability to have effective and compassionate communication, sometimes under complex circumstances
Communication and relationship skills - provides and receives complex, sensitive information with barriers to understanding communicates with patients/clients and carers, using empathy and reassurance and where there are barriers to understanding e.g. learning disabilities or language
Actively assist in administrative duties when appropriate (appointments, taking telephone queries, helping admin staff)
Engage in audit and/or information collection
Analytical and judgemental skills - Range of facts of situations requiring analysis: deciding on implementation of care programmes where there is a number of options.
Undertake training to be able to complete patient assessment, shared decision tool, assessment tools and smoking cessation
Ensure onward referral to other relevant care providers ie smoking cessation, pulmonary rehabilitation, etc
Undertake telephone lung health check appointments to establish risk scores and eligibility for further investigation
