Job summary
This is an exciting new role to support the digital delivery of pulmonary rehabilitation (PR) across two PR services based in Liverpool and Knowlsey.
Main duties of the job
This role will support the teams in engaging more patients to accept a referral into PR and support patients to identify personalised programmes of care.
The role will support patients on a digital PR pathway with weekly support calls and progression of exercise programmes. The post holder will support with education and self-management support to improve quality of life.

The assistant practitioner will function at a higher competency level than that of other rehabilitation support staff. The assistant practitioner will assist the pulmonary rehabilitation (PR) staff in the delivery of the pulmonary rehabilitation service for 2 services across the Cheshire and Merseyside footprint, these services are:

Essential
- Previous experience as a HCA, rehabilitation/therapy assistant.
- Evidence of independently managing own caseload.
- Liaising with other agencies and professions.
- Worked as part of an MDT
- Current knowledge of working within a healthcare setting
Desirable
- Community experience
- Worked previously with chronic respiratory conditions
- Experience working with palliative patients
Skills and abilities
Essential
- Be able to plan and modify treatment programmes.
- Prioritisation skills to manage own workload independently.
- The ability to understand and respect patients and carers differing points of views.
- Excellent organisational skills
- Excellent verbal communication skills
Qualifications
Essential
- Completion of AP course or equivalent clinical experience
Desirable
- IT training / skills
- Motivational interviewing training
- NVQ qualification
