An
exciting opportunity has become available within the be part of the National
Targeted Lung Health Checks Programme for the role of Band 4 Targeted Lung Health
Check Practitioner. The role will be based at Liverpool Heart and Chest
hospital.
The Targeted
Lung Health Check (TLHC) programme will enable the early detection of lung
cancer in order to save lives. Cheshire and Merseyside has been identified as one
of the higher areas of disease prevalence, in which significant impact can be
made from the recognition of early stage lung cancer and offering curative
treatment.
As a Target
Lung Health Check Practitioner, you will actively participate in undertaking
lung health checks for the eligible population, as well as providing health
education and smoking cessation.
The post
holder will be expected to effectively communicate with members of the public
from a variety of different backgrounds and be able to rely on your
organisation skills to meet the requirements of our patients, and to work
collaboratively and cooperatively with the nursing and wider multi-disciplinary
team.
This
is a permanent term post with the Liverpool Heart & Chest (LHCH) and will
be part of the National Targeted Lung Health Checks Programme, which is a key
element of the early diagnosis of cancer and part of the NHS Long Term Plan. You will work closely with nursing, admin and
radiology teams to deliver a whole system approach to improve health outcomes
and reduce inequalities by focussing on lung cancer.
Main duties of the job
Applicants must demonstrate essential requirements from the job description, key elements of this include;
- Undertake training to be able to complete patient assessment, shared decision tool, assessment tools and smoking cessation
- Undertake telephone lung health check appointments to establish risk scores and eligibility for further investigation
- Analytical and judgemental skills Range of facts of situations requiring analysis: deciding on implementation of care programmes where there is a number of options.
- Ability to have effective and compassionate communication and relationship skills, sometimes under complex circumstances
- Work collaboratively with other health professionals to ensure that patients needs are met
- Actively assist in administrative duties when appropriate (appointments, taking telephone queries, helping admin staff)
- Use the expertise and knowledge of professional colleagues, acknowledging the boundaries of different roles
- Ensure onward referral to other relevant care providers ie smoking cessation, pulmonary rehabilitation, etc
- Motivate and empower patients to address nicotine addiction by brief intervention
- Involve patients, carers and relatives in the giving of information regarding the project and in the delivery of care
- Exercise judgement, discretion and decision making in clinical care in line with the project protocol
