Job summary

LHCH is one of the largest Trusts in the UK for percutaneous coronary intervention (2500 PCIs per annum) . The TAVI programme consists of four interventional cardiologists, two aortic and one non-aortic surgeon, an imaging cardiologist and a radiologist. There are 2 dedicated TAVI specialist nurses and a co-ordinator who complete the TAVI team. Two hundred and forty TAVI procedures were performed in 2019. Transfemoral TAVI is minimally invasive and performed under local anaesthesia in the catheter lab. Complex ASD and PFO closures and other structural procedures also undertaken.

Main duties of the job

The successful applicant will work under the supervision of the consultants who perform the different structural interventions, although they will be assigned a named educational supervisor. There will be exposure to all aspects of structural intervention as well as opportunities to maintain skills in coronary intervention. Outpatient work will include a weekly TAVI clinic and occasional PCI clinics (no more than 2 clinics per week in total). The appointee will be responsible for the management of TAVI patients and also for the post-procedure management of other structural intervention patients. There will be an opportunity to gain experience in other interventions such as LAAO, PFO/ASD closure, percutaneous VSD closure.

The successful candidate will be expected to become fully competent in TAVI and other procedures by the end of their 12 months and be able to take up a consultant post which includes structural intervention. We would expect a suitably trained appointee to perform at least 80-100 first operator trans-femoral cases by the end of their fellowship. There will be regular emergency cover as part of the cadiology SpR rota which will be closely supervised by consultant staff.

JOB SUMMARY

Liverpool Heart and Chest Hospital (LHCH) provide medical and surgical adult cardiothoracic services to the people of Merseyside, North Wales, Isle of Man, parts of Lancashire and Cheshire.

This catchment area has a population of 2.8 million people.

The Centre provides a full range of invasive and non-invasive diagnostic cardiac and pulmonary facilities. Catheter Lab: Twice weekly transfemoral TAVI list, monthly transapical TAVI list, monthly structural intervention list under general anaesthesia.

Ward Work: The trainee will perform regular ward rounds for TAVI patients and will supervise the day to day work of the senior house officers/trust doctors in conjunction with the other trainees. The trainee will participate in the registrar of the week rota (1 in 15)

General Duties

On call -Tier 2 general cardiology and PPCI on-call out of hours

Ward Work - The trainee will attend one consultant ward round per week and will supervise the day to day work of the tier 1 trainees.

The appointees will be encouraged to cross cover sessions when possible to maximise training opportunities.

RESEARCH/TEACHING/ADMINISTRATION

Equivalent to two half-days per week. The post-holders will be expected to take a minimum of a half day for research/private study each week. Research participation is expected and the post-holder will be given a specific project to carry out with a view to publication within the time frame of the fellowship. Attendance at national and international meetings will be encouraged, particularly for presentation of data.

The centre undertakes teaching of all final year under graduates from the University of Liverpool Medical School and is also recognised for the training of nurses and other professional and technical staff. The post-holders will be involved in teaching these groups of staff and other junior doctors.

A regular programme of conferences, lectures and seminars takes place within LHCH. The centre is committed to the principal and practice of clinical audit in which all medical staff are expected to participate. The appointees will be expected to attend and take part in the various educational meetings. They will be expected to present cases, develop an audit interest and present the data locally.

A specialist library is available on site. In addition, the libraries of the Liverpool Medical Institution, Liverpool University and Broadgreen Hospital are also available for reference. There is also a purpose built doctors mess, including office space, computing facilities and recreational facilities.

RECORD OF TRAINING

All training is subject to annual assessment by an educational supervisor and the programme director. Educational supervisor will be assigned at the start of the fellowship and will provide close supervision to ensure training goals are obtained. As well as serving as a review and training, this will allow planning and flexibility within the post-holders programmes and will allow for career advice and development of specific interests accordingly. Although not within a formal training programme the disciplines of the usual SpR commitment will be applied.

Post-holders will be expected to keep a log book of all practical procedures undertaken, which will be regularly reviewed by their supervising consultant and signed accordingly.

ANNUAL LEAVE AND STUDY LEAVE

The post-holders will have full entitlement to annual and study leave. All leave arrangements are subject to approval by the administrative SpR and the supervisory consultant. In addition, study leave applications are also subject to approval by the Postgraduate Clinical Tutor.
