The Knowsley Community Respiratory Service provides care for people in the community and in their own home for a variety of respiratory conditions, including COPD, bronchiectasis, asthma and ILD. The team provides MDT consultant lead diagnostic and optimisation clinics, an admission avoidable service, virtual wards, pulmonary rehabilitation, home oxygen review service, smoking secession support and psychology.
Main duties of the job
This role is to provide support into the pulmonary rehabilitation service. The successful candidate will be involved in the assessment of patients for pulmonary rehabilitation and supporting in the running of pulmonary rehabilitation classes. The post holder will also support in the delivery of community walks and gym session to support and encourage more patients to engage with exercise and remain physiocally actie.

JOB SUMMARY
To work as part of the KCRS in the delivery and co-ordination of exercise based Pulmonary Rehabilitation ensuring high quality, timely and evidence-based care. To provide evidenced-based care to patients with a variety of underlying chronic respiratory disease, including but not limited to COPD, bronchiectasis, asthma and pulmonary fibrosis.
CLINICAL
To assess, plan, treat and review the exercise needs of patients referred for Pulmonary Rehabilitation with an underlying chronic respiratory disease.
To deliver the exercise element and participate in the education program of Pulmonary Rehabilitation.
To ensure that patients progress appropriately through sessions and give advice and information as necessary.
To refer patients to appropriate community support programmes.
To communicate effectively with members of the multi-disciplinary team to ensure holistic patient care.
To provide PR in a variety of clinical settings, including; leisure centres, community venues, gyms, virtual platforms, home-based and utilizing exercise prescription platforms.
To be able to work as part of a team, but also to work independently in a variety of clinical settings.
